Reception first
This The Sinking City 2 Akeley Hospital walkthrough starts at reception, and you should clear it properly before going deeper. There is an ERC crate here, and it is one of the easiest guaranteed inventory-upgrade sites in the middle of the game. If you do not have an activator pen yet, note it and come back — inside a chapter you can backtrack freely.
The wings, the prisms and the loop
The hospital is a spine with wings off it: east and west, the morgue, the pharmacy, the chapel and the isolation wards. The prism stations gate movement between those blocks, which is why the chapter loops — you clear a wing, find a station, and it opens a door you walked past twenty minutes ago.
The rule at every station is the same: insert the Prism, rotate until the coloured symbol matches the marker of the same colour, confirm, and take the Prism with you. Blue, yellow and pink are three separate locks, not a sequence.
The two keys and the acid
Three items open the rest of the hospital:
- Vertebral Key — one wing door.
- Caduceus Key — a second, and the route to later mask pieces.
- Anti-Rust Acid — for corroded locks. If a door refuses both keys, acid is the answer.

The mask puzzle
Five pieces: the Woman's Face Mask, the right and left face halves, the Old Man's Face Mask and the Head Surgeon's Face Mask. The fixture gives no useful feedback until you hold all five, so partial progress reads as a broken puzzle when it is really an incomplete collection.

The Shoggoth
The chapter's boss is a mechanic fight rather than a damage race. Activate all three devices in the arena to force the Shoggoth to open its eyes, then shoot the eyes. Repeat; its assault escalates with each phase.
Go in with ammunition. If you are short, drop the difficulty for the fight — it is changeable from the pause menu at any time and costs you nothing on the trophy side. The full fight breakdown is on the Shoggoth page.
Before you leave the hospital
- The reception ERC crate.
- Every prism station — one of them opens a wing you may never have entered.
- Both keyed wings, plus anything the Anti-Rust Acid opens.
